3.Bilateral Knee Coverage Tissue Reconstruction with Perforator Fasciocutaenous Flap in A Child

A 5-year-old male patient came to our hospital with injuries on both knees. He got his knees injured when he was clinging to the back of a moving van and his knees were rubbing against the ground. The wounds were sutured primarily in an emergency room. When separation started on both the sutured areas, the parents decided to come to us. After a couple of days of wound care and debridement, we operated the patient on both knees following the same procedure with 1-week interval.
The bilateral perforator fasciocutaneous island flaps were elevated using descending genicular artery, and then the flaps were rotated by a 90° angle on its pedicle area. The donor area was repaired primarily and closed up partially by a split thickness skin graft. Our postoperative observation showed that the reconstruction allowed the patient to sit up and sit down comfortably.

5.Vancomycin Resistance Among Methicillin-Resistant Staphylococcus Aureus Isolated from Clinical Samples in Erbil City-Iraq

Bacterial isolates obtained from different sources of 348 human specimens including burn, wound, urine, and stool from the database of Internal Lab of Teaching Hospital, Irbil-Iraqi Kurdistan region, were collected from May 20, 2012, through January 19, 2013, of which 228 isolates were positive for Staphylococcus aureus.
Cultural studies were performed using different cultures and biochemical tests to ensure the identity of species under study. The susceptibility of the isolates for the antibiotics test were done using 22 different antibiotic disks including carbenicillin (CAR), vancomycin (VA), clindamycin (DA), methicillin (MY), cephalothin (KF), pipercillin (PRL), nitrofurantoin (F), cephalexin (CL), rifampicin (RA), gentamycin (G), chloramphenicol (C), trimethoprim – sulfamethoxazole (SXT), ceftazidime (CAZ), polymyxin B (PB), amoxicillin–cluvalinic acid (AMC), doxycycline (DO), amikacin (AK), oxacillin (OX), ciprofloxacin (CIP), cefixime (CFM), cefoperazone (CEP), and neomycin (NEO).
The results showed that resistance for the antibiotics ranged from 26.31% to 98.61% for DA and MY, respectively. A total of 78.94% of the isolates that demonstrated resistance to MY were also found to resist VA. Thus, we conclude that some strains of S. aureus isolates acquired genes that are able to resist those antibiotics.

Although blood transfusions are life sporting measures, it can cause alloimmunization, iron loading, bone marrow suppression, and infections etc. Therefore, it should be used according to patients' needs and blood groups should be dtermined prior to the first transfusion.

7.Evaluation of Liver Biopsies in Children

Percutaneous liver needle biopsies were carried out on 947 children with the age range of 3 hours to 17 years. Vim-Silverman needle was used mainly for the microscopic examination of the organ. It is proved to be safe, easy, and adequate method for the architectural examination of the liver.
